This role ensures a positive membership experience by coordinating clinical care, assisting with patient education, and supporting front-office operations when needed. The RN promotes MDVIP’s values by fostering long-term patient relationships, contributing to membership growth/retention, and maintaining compliance with clinical standards.Performance in this role will be measured by:Patient satisfaction and retention scoresTimely and accurate completion of clinical documentation (encounters closed within 48 hours)Compliance with MDVIP and regulatory guidelines (HIPAA, OSHA, CLIA, etc.)Effective coordination of care and communication with patients and physicianContribution to practice efficiency (reduction in wait times, streamlined workflows)Implementation and management of Chronic Care Management (CCM), including accurate documentation, patient enrollment, monthly call compliance, and revenue contributionSupport in achieving practice membership growth and service utilization goalsEssential Duties and Responsibilities / % of timeThe Registered Nurse (RN) will perform the following functions (other duties may be assigned as needed):Clinical Care & Patient SupportProvide direct patient care including vital signs, medication administration, injections, immunizations, wound care, and diagnostic testing within scope of practice.Prepare patients for examinations and procedures, ensuring comfort and understanding.Perform patient assessments and document clinical findings accurately in the EMR (AthenaNet).Coordinate follow-up care, referrals, and preventive screenings.Educate patients on disease management, medication use, and wellness strategies.Chronic Care Management (CCM)Identify eligible patients for CCM enrollment in alignment with CMS and MDVIP guidelines.Conduct and document monthly CCM calls, addressing ongoing health needs, medication adherence, and care plan goals.Collaborate with physicians to develop and update individualized care plans.Track, document, and report CCM services in compliance with billing and regulatory standards.Serve as the primary patient contact for CCM, reinforcing MDVIP’s personalized care model and continuity of care.Membership Experience & RetentionAssist physician in delivering personalized care aligned with MDVIP’s model.Support membership retention by building relationships with patients and providing exceptional service.Participate in wellness initiatives, executive physical, and advanced patient services.Administrative & Operational SupportAssist with scheduling, phone triage, and patient communication when needed.Verify patient insurance, demographics, and eligibility.Participate in quality improvement initiatives and compliance audits.Maintain accurate logs for vaccines, CLIA-waived tests, and other regulatory requirements.Team CollaborationWork closely with physicians, medical assistants, and practice coordinators to ensure seamless care delivery.Assist with onboarding and mentoring of new staff as needed.Support MDVIP practice management initiatives and projects.Knowledge/Skills:Proficiency with EMR systems ; Athena or EPIC experience preferred - location dependent. Strong clinical assessment and triage skills.Excellent communication and patient service orientation.Ability to manage multiple priorities in a fast-paced environment.Knowledge of regulatory standards (HIPAA, OSHA, CLIA).Computer Proficiency:Proficient in Microsoft Office applications.
