You’ll provide support, guidance, and resources, helping patients get the care they need while connecting them to the community services that make life easier.What You’ll Do:Offer crisis support, counseling, and resource referrals for patients and familiesHelp plan safe discharges and transitions of careCoordinate with healthcare teams to keep patients supported and informedConnect patients to community resources for housing, food, transportation, and moreShifts Available:M-F Full Time DaysPRN/Casual Days and WeekendsPart Time Days, Every 5th WeekendSat/Sun/Mon or Fri/Sat/Sun***depending on shift, eligible for up to a $5,000 sign-on bonus, $10,000 in student loan reimbursement, and shift differential***Bachelor's BSW or Master's Degree MSW (or willing to enter a Master's Degree program) from a school of Social Work accredited by the Council of Social Work Education
