The Community Health Administration (CHA) works to prevent the leading causes of death, protect and promote the health of mothers and children and eliminate racial and ethnic health disparities in health. CHA focuses on nutrition and physical activity promotion; cancer and chronic disease prevention and control; access to quality primary health care services; and the health of families across the lifespan. CHAs approach targets the multiple factors that influence health through evidence-based programs, policies and systems change.The Early Childhood Health Division (ECHD) works to ensure that all children and families have access to a continuum of comprehensive, high-quality early childhood programs and services that promote child well-being and school readiness and ensure that all children are healthy, ready to learn and have safe passage through the early years. ECHD programs include Help Me Grow DC, Smart from the Start, Healthy Steps, Children and Youth with Special Health Care Needs (CYSHCN), and Home Visiting programs, including SafeCare Augmented (SafeCare). SafeCare is an in-home behavioral parenting program for parents and caregivers of children ages 0-5 years that aims to promote positive parent-child interactions, informed caregiver response to childhood illness and injury, and a safe home environment. The incumbent is responsible for planning and conducting weekly home visits for families enrolled in SafeCare, administering screenings and assessments, maintaining an assigned caseload and providing families with resources, information, and referrals aligned with family needs.Position Responsibilities:Responsible for coordinating, planning and implementing weekly home visits for families, ensuring timely and efficient service delivery while maintaining clear communication with clients to support family engagement and attainment of program goals.
