The role includes monitoring vital signs, administering medications, performing wound care, and assisting with activities of daily living. The LPN ensures patient safety, adherence to care plans, and optimal outcomes in accordance with established nursing standards and hospital policies.Essential FunctionsProvides direct patient care, including monitoring vital signs and assisting with ADLsAdministers medications as prescribed within scope of practiceDocuments patient care, assessments, and interventions accurately and timelyAssists with specimen collection, wound care, and other clinical procedures as directedCollaborates with the RN in developing, updating, and implementing patient care plansMonitors and reports changes in patient condition to the RN or physicianProvides patient and family education regarding care plans and discharge instructionsMaintains a safe and clean patient environment in accordance with infection control protocolsParticipates in staff training, competency assessments, and quality improvement initiativesResponds to emergencies and assists in patient stabilization as directedKnowledge/Skills/Abilities/ExpectationsKnowledge of basic nursing principles, medication administration, and patient care proceduresStrong communication and interpersonal skills for interacting with patients and the healthcare teamAbility to work effectively in a fast-paced, high-acuity environmentProficiency in electronic medical records (EMR) systemsDemonstrates professionalism, accountability, and a commitment to patient-centered careAbility to lift and move patients and equipment as necessaryStrong organizational skills and attention to detailQualificationsEducationGraduate of an accredited Practical Nursing Program requiredLicense/CertificationsCurrent state LPN/LVN licensure requiredBasic Life Support (BLS) Certification as required per facility policyDe-escalation Training as required per facility policyExperienceMinimum of six (6) months of experience in a medical/surgical setting preferred
