Day Habilitation offers several activities to choose from and selected candidate should be able to instruct those in gardening, cooking/baking, music appreciation, self-advocacy, social skills, fitness, pet care, budgeting, work skills development, arts and crafts, reading, clothing, sensory activities and activities of daily living.It is the philosophy of Day Habilitation that courses offered will be integrated in the least restrictive environment and in the community setting as much as possible. Therefore, the majority of the Day Habilitation will not occur u201cwithin the wallsu201d of the program.Some of the duties of the selected candidate can include:Person will be responsible for scheduling outings, Day Hab paperwork, ensuring the safety and welfare of the individuals, completing Day Hab billing and documents in a timely manner and engaging individuals in activities and work-related activities. Selected person will supervise Direct Support Professionals at the site and function as the back-up to the Day Hab Coordinator. Completion of other assigned tasks based on the operational needs of the site location. Additional duties may include:Oversight of a program location with responsibility to ensure compliance with regulatory requirements in the areas of Day Habilitation, fiscal responsibility, and incident management. They should possess excellent communication, writing and organizational skills and be able to work independently. Review of the monthly summary notes for each individual ensuring all required components are included to meet the Medicaid billing requirements. Train day program staff on u2013 the composition of monthly summary notes as needed and the individualu2019s Safeguard Summary Plans to always ensure health and safety. Monitor implementation of Safeguard Summary Plans, Day Program Staff Action Plans and Support and Risk Accountability Plans. Review and revising as needed to ensure that plans continue to reflect the individualu2019s abilities and need for protective oversight and necessary supports. Review of Clinic Referrals for PT, OT, Nutrition, Speech, Psychology and other Article 16 services. Participate with other team members in a variety of individual, team and continuous improvement meetings and forums as scheduled to promote comprehensive and individualized services. Interact with families, Care Coordinators, and other service providers as needed. Participate in Adjunct tasks as assigned. May at times be the lead clinical person in an assigned room responsible for the day-to-day operations of a Day Hab room.They will be responsible for scheduling outings, Day Hab paperwork, ensuring the safety and welfare of the individuals, completing Day Hab billing and documents in a timely manner and engaging individuals in activities and work-related activities.
