Employment Type: Full time Shift: Day Shift Description: LPN - Saratoga/Washington Counties full time weekdays 8am to 4pm (no weekends and no mandatory overtime) The Community Hospice has a rewarding opportunity for an LPN to work as part of our team caring for patients and families in their homes primarily in the Saratoga/Washington county area. What you will do: + Coordinate closely with our RN Supervisor and RNs to provide nursing and personal care services as a part of a comprehensive plan of care to provide added support to patients and families + This is an ideal opportunity for someone who enjoys working closely with a team (our teams include RNs, Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers). + Training provided in end of life care. What you will need: + Valid NYS LPN License + Minimum 1 year LPN experience required + Prior hospice/ home care experience preferred, but not required + Strong computer skills + Organization and communication skills and ability to work as part of a team Community Hospice provides a thorough orientation, preceptors and a supportive work environment. No On-Call, No Weekends, No Mandated Overtime _Pay Range: $24.00-$30.10_ _Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
