Summary The Clermont VA CBOC located at 3200 Citrus Tower Boulevard Clermont FL 34711 is looking for a Licensed Practical Nurse to work in Patient Aligned Care Team (PACT). The PACT LPN carries out the most complex practical nursing care assignments which include performing nonstandard and specialized activities sufficient to solve a range of complex nursing problems. The LPN uses a systematic problem-solving process in the care of multiple patients with predictable health care needs. Responsibilities VA Careers - Licensed Practical Nurse: Total Rewards of a Allied Health Professional The LPN may contribute to the plan of care by collaborating with interdisciplinary team members, the patient and the patient's family. The LPN collects data and information, recognizes changes in conditions and reports this to the RN supervisor or another appropriate clinical supervisor to assist in the identification of problems and formulation of goals, outcomes, and patient-centered plans of care that are developed in collaboration with patients, their families, and the interdisciplinary health care team. Demonstrates competence in delivering and improving holistic care through collaborative strategies with others. The GS-7 LPN has an exceptional degree of knowledge and skills necessary for self-direction in carrying out the most complex practical nursing care assignments for both inpatient and outpatient care. Assignments include performing nonstandard and specialized activities sufficient to solve a range of complex nursing problems of considerable difficulty and responsibility, under very general supervision. In addition to meeting the requirements for the LPN GS 3-6 levels, possess broader knowledge and skills for increased involvement in the development and modification of the Veterans plan of care, ability to exercise independent technical judgment, and possess and apply a comprehensive working knowledge of the principles, art, and science of practical nursing. Duties included but not limited to: Successfully and consistently demonstrates the exercise of independent technical judgment (without the need for readily available supervision) as well as a comprehensive and thorough working knowledge of the most complex or difficult practices and procedures. Such individuals may also serve as leaders or co- leaders in a nursing unit or for a specifically identified group of Veterans Demonstrates a broad knowledge of the Veteran treatment process and exercises judgment that demonstrates the ability to effectively intervene when appropriate. At this grade level, the LPN is expected to participate to a greater degree in prioritizing and as appropriate, modifying patient care treatment Exercises skill in influencing and communicating with unusually difficult to care for or communicate with Veterans, families, and/or caregivers who may exhibit problems with lack of self-control, resistant or abusive behaviors, or impediments in their ability to understand or follow instructions Supports the RN and/or Provider in monitoring the practice of unlicensed nursing or non-medical staff in Veteran care delivery. Promptly reports any deviations from normal practice patterns to the RN and/or Provider. In addition to responsibilities at the lower grade level, the LPN is responsible for monitoring the practice of non-licensed nursing and medical staff Work Schedule: Monday to Friday 0800-1630/ Flexible to meet organizations needs Compressed/Flexible: Not available Recruitment Incentive (Sign-on Bonus): May be authorized for highly qualified individuals Pay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
