JOB PURPOSE: Provide physical therapy treatments to PruittHealth Home Health patients in their place of residence under the supervision/direction of a physical therapist. KEY RESPONSIBILITIES: u2022 Assist with the development and implementation of the plan of care within goals defined by the physician, patient/patient family and interdisciplinary team. u2022 Helps maintain continuity of care by collaborating with the interdisciplinary team and attending physician to develop, integrate, and manage the patient care plan of care. u2022 Demonstrates intermediate knowledge of home care and competency in discipline for specific patient care skills, required for the provision of care. u2022 Meets agency productivity standards and utilizes time and resources effectively and efficiently. u2022 Completes documentation timely, accurately, and at the point of care, according to industry standards. u2022 Assist the physical therapist in assessing the physical disability and level of function of patients. u2022 Maintains current license, certification, and clinical records to meet federal, state and agency regulations and guidelines u2022 Participate in center/agency surveys (Licensure / JCAHO), and any subsequently required reports. u2022 Accepts responsibility to report any alleged or suspected violations to supervisor and/or others until the issue is resolved. u2022 Ability to be self-directed, prioritize, manage time appropriately, and meet outcomes and strategic goals by targeted deadlines. LICENSURE, CERTIFICATION AND EDUCATION REQUIREMENTS: u2022 Must have a valid and unrestricted professional license in state of practice. u2022 Graduate of an accredited Physical Therapy Assistant program. u2022 If foreign-trained, the program must be deemed substantially equivalent to a Physical Therapist Assistant entry level education in the U.S. by a credentials evaluation organization approved by the APTA. u2022 Current CPR certification. REQUIRED SKILLS AND EXPERIENCE: u2022 Minimum of 2 years' therapy experience. u2022 Valid driveru2019s license and automobile liability insurance.
