Services may include, but are not limited to:Coordinating with community providers tofacilitatecomprehensive care and referrals for familiesParticipating in collaborative case planning and case review meetings, when requestedDelivering case management activities such as:Supervised visitationParent aide supportBehavioral AideTransportationCrisis interventionAdvocacy and linkage to services (including drug screening facilitation if applicable)Completing in-home visits tomonitorprogress and provide ongoing support for families involved with DFCS.Offering parenting education and modeling positive parenting strategiesCoordinating with external professionals (e.g., schools, therapists, case managers) to ensure service alignmentMaintaining documentation of servicesrendered, including case notes and service logs,in accordance withcontractual requirements and applicable standardsProviding transportation services related to the family service plan (e.g., visitation, appointments) as appropriateBachelor’s degree in a human services-related field (e.g., Social Work, Psychology, Counseling, Criminal Justice)orHigh School Diploma/GED with at least 5 years of verifiable experience in the fieldStrong understanding of family systems, trauma-informed care, and community resourcesReliable transportation, valid driver’s license, and proof of insuranceWillingness to complete required background checks and contractor onboardingDeliverablesTimely submission of service logs, progress updates, and reportsAccurate documentation aligned with billing and reporting standardsServices providedin accordance withcontract terms, including hourly or task-based invoicingIf you share our commitment to empowering families and delivering trauma-informed support, we invite you to explore a partnership opportunity with Health Connect America.
