If you have the required skill set, education requirements, and experience, please click the submit button and follow for next steps.The Care Connector is responsible for supporting pregnant and postpartum members. , The Care Connector will facilitate and coordinate care for maternity members and will serve as a single point of contact in the Bright Start Maternity Program. , The Care Connector assesses maternity members to determine care coordination needs, identifies problems/barriers, and addresses member issues/concerns. , Implements the appropriate level of intervention based upon the memberâs needs, schedules follow up calls as necessary and makes appropriate referrals. , Coordinates care and services with member, providers, and Community Based Organizations as appropriate.Work ArrangementRemote, must reside in Pennsylvania to be consideredEducation/Experience:Must be bilingual in SpanishHigh School/GEDMinimum of 2 years previous work experience, preferably in a health care settingManaged care experience preferred., Medical terminology/documentation experience is a plusPrior telephonic customer service experience desired, Skills ,AbilitiesExcellent interpersonal and verbal/telephonic communication skills and the ability to write clear and concise documentation.Attention to detail and confidentiality with strong organizational skillsProficient PC skills in a Windows based environment including Outlook and Teams, Your career starts now.
