You will play a pivotal role in shaping the future of patient care for our Heart Transplant program!Position OverviewThe Nurse Navigator is responsible for the coordination of patient care across the continuum within the scope of nursing practice. The navigator educates/provides information and supports patients to guide and facilitate understanding of treatment plans. The Nurse Navigator oversees, directs, and provides holistic, culturally competent, and evidence-based nursing care.
