If youre passionate about preventing homelessness and creating real, lasting changethis is your opportunity to make a meaningful impact every single day.This position is located at Conway Health and Resource Center in Southwest, DC.Our Approach and Values:- We celebrate peoples strengths and acknowledge the impact of trauma on peoples lives.- We embrace diversity, welcome all voices, and treat everyone with respect and compassion.- We lead and advocate for changes to make systems more equitable.- We strive for excellence and value integrity in all that we doWhat Youll Do- Works with adults and children to develop case management plans that are updated at least monthly and signed by clients and case manager, which incorporate goals for both adults and children related to core goal areas: finding stable housing; increasing income; and family stabilization and support.- Meets weekly and as needed with families on caseload to develop case plans, review progress, and to follow-up on tasks and services.- Coordinates with available employment supports to ensure that clients employment and educational needs are being effectively addressed.- Networks with community resources for housing, employment, substance abuse treatment, mental health treatment, training, etc., and makes referrals as appropriate, including to COH Therapist and other community resources, as appropriate.- Completes required assessments with families.- Maintains regular communication in accordance with program contract guidelines with all families on caseload and documents progressive engagement efforts with difficult to engage families.- Obtains signed release of information forms and communicates regularly with staff at treatment programs, substance abuse programs, and vocational programs RequirementsMust-Haves- Bachelors degree in social work or related field required or 4 years social service experience.- Minimum one year work experience in social service or related field.- Strong interpersonal, conflict-resolution, and organizational skills.- Ability to work independently and as a team.- A valid drivers license and regular access to a car, as well as proof of auto insurance, required for staff working in scattered site housing programs.Nice-to-Haves- Prior experience working with individuals or families experiencing homelessness and/or living in poverty strongly.Why Youll Love Working HereAt COH,we prioritize the following well-being and work-life balance-centered benefits: (Full-Time)- 8 x Washington Post 150 Top Workplaces winner- 8-hour workdayswith paid lunch- 3 weeks vacation (additional week after two years), 2 weeks sick leave, + 11.5 paid holidays and one personal floating holiday on an annual basis.- Annual performance-based raises, up to 5% of your annual pay.- Tuition reimbursement and loan repayment (NHSC and DCHPLRP), Licensing reimbursement and CEU funding.- Medical, dental, vision, life and disability insurance + 403(b) retirement.- Leadership development, internal promotions and career growth opportunities.- A culture grounded in equity, compassion, and well-being.About UsCommunity of Hope is a mission-driven, innovative, rapidly growing nonprofit, and Federally Qualified Health Center.
