Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Responsibilities Total Rewards of a Allied Health Professional Major Duties/Functions include, but not limited to: Assists with patient care problems and deviations in accordance with VA Nursing Practice and LPN licensure standards. Maintains knowledge and skill to care for a wide range of patients, including those with chronic and acute illnesses. Demonstrates comprehensive understanding of disease conditions, nursing principles, and standards/practices to coordinate nursing care required in the clinic setting and alter the sequence and/or care delivered to meet individual patient's needs. Triages, records, and reports vital signs, weight, and blood glucose levels. Appropriately prioritizes patient needs as routine, urgent, or emergent in the clinical setting . Check's patients into the clinic, interviews patient and/or significant other, and documents intake data as per policy. Possesses comprehensive knowledge of VA protocols and VHA directives related to clinic management, documentation, and computer technology. Demonstrates knowledge, skill, and ability to promptly recognize and effectively intervene in potentially urgent or emergent situations. Completes Clinical Reminders to meet the changing needs of the patient with notification to the RN or provider for concurrence. Recognition of urgent/emergent patient care needs, including hypo/hyperglycemia, acute shortness of breath, acute chest pain, seizures, stroke symptoms, mental health issues, etc. Triages prior to provider face-to-face visits, nurse visits, telephone triage, secure messaging, etc. Applies knowledge by considering the interrelationship between age, physical, emotional, social, cultural, and population-specific needs when providing care. Communicates calmly, courteously, and professionally with all patients, personnel, and visitors while maintaining confidentiality in accordance with HIPAA regulations and Medical Center policies. Uses advanced skills to communicate with verbally or physically abusive patients or visitors using Prevention and Management of Disruptive Behavior (PMDB) principles and policy guidelines. Collaborates with the PACT team and other interdisciplinary services in the development, implementation, and evaluation of patient care. Demonstrates knowledge and adherence to identified standards of care/practice for a variety of medical diagnoses. Collaborates with PACT-assigned Home Tele-Health personnel to address patient care needs in a timely manner and to transition care from Home Tele-Health back to the PACT team. Completes closure of patient encounters in a timely manner (within 24 hours of patient visit). Demonstrates appropriate designation of patient service connection, diagnosis, and procedures. Possesses knowledge and ability to access open encounter report in VISTA and collaborate with NM, Clinical Informatics, and DSS (when needed) to properly close encounters for timely billing and reimbursement. Provides literature to patients/families about preparation for procedures and/or diagnostic tests. Collaborates with the PACT team and other interdisciplinary services in the development, implementation, and evaluation of patient care. Demonstrates knowledge and adherence to identified standards of practice for a variety of medical diagnoses. .
