Job Summary This position will provide case management services to families eligible to participate in the YMCA Child Placing Agency Program and Behavior Rehabilitation Services Program. This will include supporting YMCA licensed caregivers of youth in foster care in the King County, Snohomish County, and Pierce county areas, youth enrolled in intensive in-home stabilization services, and youth enrolled in therapeutic foster care services. Case management services include coordinating or providing services to enable the child and caregivers to utilize medical, educational, social, clinical, and other services necessary for maintaining the child's physical, psychological, and/or developmental health. You will also provide leadership and support to the greater Case Management team and ensure that all clients in the CPA and BRS programs are receiving quality case management services. This position is at the Social Impact Center Y, the social services branch of YMCA of Greater Seattle (YGS). The mission of the Social Impact Center Y is to accelerate young peopleu2019s ability to build safe, successful, and happy lives. Every year, we serve 5,000 children, youth, young adults, and families across King County with programs that aim to strengthen young peopleu2019s ability to develop to their fullest potential through four strategies: reducing risk factors; navigating systems towards successful life transition; ensuring stability, safety and wellness in times of crisis; and building competencies and social capital. The Social Impact Center Yu2019s programs include housing, behavioral health, foster care licensing, violence prevention, education, and employment. Many of the young people we serve have had involvement in the foster care, criminal justice system, and/or homelessness systems. Position Type/Expected Hours of Work Monday-Friday 9a-5p with flexibility for evenings and weekends as needed to meet program and participant needs. Some benefits only available to full-time staff Hiring Range: $24.00 u2013 $27.00/hr Responsibilities + Provide case management and care coordination services, as outlined in the CPA and BRS Contracts, Statements of Work, Handbooks, and Policy/Procedures, to clients enrolled in the CPA and BRS Programs including, licensed and unlicensed caregivers of youth in foster care, therapeutic foster care, intensive in-home stabilization services, and Outpatient Mental Health services. + Maintain a caseload size in accordance with contractual requirements and guidelines. + Ensure clients receive all required screenings, assessments, services, interventions, and opportunities needed to fully meet their physical, psychological, social, emotional, relational, developmental, educational, independent-living, housing, and permanency needs in a timely and effective manner. + In coordination with the Program Director, and in collaboration with the child & family team, ensure clientsu2019 transitions are well coordinated and facilitated, including placements & discharges, and ensure all required intake, service planning, and discharge/exit documentation is completed fully and on-time. Provide leadership and guidance in the placement referral process, and ensure that case transfers (of CMs, Intervention Specialists, and Child & Family Therapists) are smooth and as least disruptive as possible to the client. + Ensure clientsu2019 parent/child/sibling visitation needs and requirements are met. Ensure natural supports and life-long connections are developed and stabilized. + Coordinate and ensure inclusion of the clientu2019s Child/Family Team in the planning and decision-making process. + Ensure foster parents/caregivers receive the necessary support services (case-management, care coordination, respite, resources, etc) in order to fully meet the clientu2019s needs. + Ensure all services provided are well documented in the clientu2019s file in accordance with all WAC and contractual requirements and guidelines. + Ensure all contract required client progress reviews and reports are completed thoroughly and accurately, are well written, and submitted on-time. + Ensure client Referral Agreements and Exceptional Cost Plans are developed to fully represent the clientu2019s needs; in accordance with policy/procedure; receive required approvals; and are submitted and renewed on-time. + Ensure each clientu2019s file/record is fully in compliance with WAC, DSHS/DLR, DSHS/CA, DSHS/DBHR, KC-BHO, COA Accreditation, and YMCA requirements. + Ensure all clientsu2019 information is kept confidential and only released in accordance with HIPAA, DSHS, and YMCA policy and procedure. + Conduct Intake Assessments + Create Individual Service Plans or Caregiver Support Plans clients. Monitor treatment progress and review plans on a quarterly basis. + Participate in clinical consults for clients as necessary in order to be able to provide clinical advocacy for clients. In conjunction with health professional, assess clientsu2019 mental health needs and determine appropriate level of care and support. + Other duties as assigned. Code of Conduct for Applicants ( Qualifications u2022 Bacheloru2019s degree in social work, social services, or a closely related field from an accredited school. u2022 Two or more yearsu2019 experience in providing social services to youth and families, preferably in mental health and child welfare settings. u2022 Knowledge of crisis de-escalation. u2022 Ability to teach effective independence skills for a variety of learning styles and to plan supportive activities. u2022 Strong youth engagement and relationship-building skills. u2022 Strong verbal and written communication skills. u2022 Basic computer skills and working knowledge of Microsoft Office. u2022 Ability to transport self and client(s) in a safe and effective manner, and to meet clients at community locations; proof of adequate vehicle insurance coverage is required, or ability to travel to areas without public transportation. u2022 Current state approved First Aid and CPR certification. u2022 Current approved HIV blood borne pathogens training. u2022 Ability to pass a DCYF Criminal Background Clearance Within 30 days of employment or first available training. Personal Vehicle Authorized Driver Requirement This role transports clients as an u201cauthorized driveru201d for the YMCA of Greater Seattle (YGS) using your own vehicle (proof of current auto insurance and vehicle registration are required). Driving duties for this role are Required. Driving roles are safety-sensitive. To become an authorized driver, you must be 21+, have possessed a driver's license for at least 3 years, and pass a drug screen that will test for impairing substances (including marijuana) in addition to meeting the driving record standards below: YGS Driving Record Standards Within the last three (3) years: No more than two (2) vehicle related suspension(s) and/or reinstatement(s). No more than two (2) moving violations and/or accidents.
