As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants ofRochester, NY (and in Canandaigua, N.Y. Jordan Health is now seeking aCommunity Health Workerwho, under the guidance of the Director of Social Emotional and Mental Health, is primarily responsible for serving as a bridge between healthcare providers, behavioral health resources, and the community. This role provides support, education, and navigation services to individuals facing health challenges, utilizing a u201cwhole personu201d approach. This position also aims to improve access to healthcare and behavioral health services, enhance community wellness, and reduce health disparities. Requirements TheCommunity Health Workerwill ensure to: u00b7 Improve access to behavioral health services u00b7 Support individuals in navigating the healthcare system u00b7 Facilitate coordination of care and community collaboration u00b7 Maintain accurate records and ensure effective service delivery u00b7 Address social determinants of health Education And Experience Required: u00b7 High School diploma or GED equivalent required u00b7 Associates degree, or higher, in social work, psychology, public health, or related field preferred; equivalent experience considered. u00b7 Experience in community health, case management, or behavioral health services. u00b7 Strong knowledge of community resources and healthcare systems. Certifications And Licenses u00b7 BLS through the American Heart Association u00b7 NYS Infection Control Certificate Special Skills, Knowledge Required: u00b7 Ability to work with diverse populations and provide culturally responsive support. u00b7 Excellent communication and interpersonal skills. u00b7 Bilingual skills preferred but not required u00b7 Ability to within the community and conduct home visits as needed.
