Clinical Social WorkerPosition: Clinical Social WorkerSalary: $75,000+/annuallyLocation:South Orange, NJJob PurposeA CSW at our practice provides interventions to remove psychosocialbarriers to health and wellness. Collaborates with members of the healthcare team, theclient/family, the Case Manager, the Daily Living Skill Counselor, and community liaisons toassure clients’ psychosocial and socioeconomic needs are addressed whereverpossible. Advocate for and empower clients to maintain a level of control over theirhealth and wellness and daily life.Essential Functions• Provide clinical services to clients that are specific, measurable and are providedbased upon intervention plans and action steps. Provide psychosocial counselingto clients• Provide close monitoring of clients through follow-up contacts and coordinateservices with other providers as necessary• Provide crisis intervention; assess and coordinate protective service issues.• Ensure that all required documentation is completed in a timely and accuratemanner, including all progress notes, treatment plans, and treatment plan reviewsincluding any assessments• Advocate for and empower clients to maintain a level of control over their health and wellness and daily living skills• Establish short and long terms goals for clients• Provide and maintain educational materials• Maintain medical records and all other care-related documentation• Complete all competency-based training, both internal and external, to qualify foror maintain licensureCompetencies1. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.• Must be available for coverage on some weekends and after 5 pm when required.• Must hold a high school diploma• Valid New Jersey driver's license is required• Must hold CPR/First Aid Valid Certification• Successful clearance of NJ background check• Successful clearance of Drug Screening• Has fulfilled the necessary training requirements and is in good standing in order to bill Medicaid for Support and Community Care Waiver (CCW) Services and CDSCompensation / Pay Rate (Up to): $70,000.00 - $80,000.00 Per Year
