As the person who is a Care Navigator, you are the vital link in the patient's journey, the person who connects the dots and ensures a smooth 'handoff' as they recover. You'll work with a diverse team of physicians, nurses, and social workers to coordinate care, educate patients and families, and ultimately, prevent unnecessary hospital readmissions.
