West Palm Beach VA Medical Center is recruiting for a Physician (Geriatrics). The Geriatrics & Extended Care Service within the West Palm Beach VA is geared toward providing direct patient care to Veterans in both inpatient and outpatient settings. Responsibilities This is an OPEN CONTINUOUS ANNOUNCEMENT. Eligible applicants will be referred until all vacancies are filled. Within Geriatrics & Extended Care Service, an array of services may be rendered in the following settings: Community Living Center (CLC), Hospice & Palliative Care, Home Based Primary Care, Blind Rehabilitation Center and Outpatient. Physicians in the Service provide direct patient care, and also serve as a role model for peers, Advanced Registered Nurse Practitioners, Physician Assistants, Residents and Medical Students. Physicians in Geriatrics & Extended Care Service will assume the following duties and responsibilities: Provides comprehensive assessment, diagnostic work-up and clinic intervention, including comprehensive patient assessment; health history, physical examination, and patient education to veterans in the CLC setting. Functions as an integral part of the interdisciplinary team to coordinate, facilitate, and provide patient care and services. Initiates appropriate referrals and consultations to other clinical services. Order and evacuate diagnostic tests as appropriate, recommend treatment and/or referral. Validate the effectiveness of previously implemented medical treatment. Initiate, evaluate, and change orders for medication. Include patient and family in planning care and establishing realistic measurable goals for health maintenance/improvement. Follow Performance Measures guidelines, including completion of clinical reminders in CPRS, access and timelines standards, prescriptions order entry, National VA formulary requirements, and customer service standards. Demonstrates flexibility in responding to clinic demands, adhere to personal and professional obligations and responsibilities, practice and maintain ethical standards, and sustain a work environment that is harmonious and customer service oriented. Participates in educational activities related to the delivery of services to community living center residents and promotes and supports appropriate continuing education programs for other staff members. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.
