Quality Control / Quality Improvement SpecialistCompany: Quality Health Care Services LLCLocation: Columbus, OH (hybrid; travel to client homes and field offices required)Employment Type: Full-timePosition SummaryLead and manage the agency's continuous quality improvement and compliance activities. Conduct audits, analyze performance data, develop corrective action plans, train staff on documentation and regulatory requirements, and report outcomes to leadership to ensure safe, effective, and survey-ready care delivery.Key Responsibilitiesu2022 Perform routine and targeted chart, documentation, and process audits for clinical and non-clinical services.u2022 Track and analyze KPIs (readmissions, medication errors, timely visits, documentation accuracy); produce regular performance reports.u2022 Lead root cause analyses and develop, implement, and monitor corrective and preventive action plans (CAPAs).u2022 Conduct pre-survey readiness reviews and support the agency during regulatory surveys and audits.u2022 Provide education, coaching, and feedback to clinicians and administrative staff on documentation best practices and compliance.u2022 Maintain and refine quality tools, audit forms, trackers, and dashboards; manage data collection and trend analysis.u2022 Investigate and resolve client complaints related to quality or billing; coordinate corrective steps and communication.u2022 Assist leadership with policy and procedure development to strengthen quality, safety, and compliance.Minimum Qualificationsu2022 Bachelor's degree in Nursing (BSN), Healthcare Administration, Health Information Management, or related field; OR RN with demonstrated quality improvement experience.u2022 2--4 years' experience in home health, hospice, long-term care, or healthcare quality assurance.u2022 Strong working knowledge of CMS conditions of participation, state regulations, and survey/accreditation standards.u2022 Proven audit experience, data analysis capability, and corrective action implementation.u2022 Proficiency with EHRs, Excel, and basic dashboarding/data visualization tools.u2022 Excellent written and verbal communication, presentation, and coaching skills.u2022 Valid driver's license and reliable transportation.Preferred Qualificationsu2022 Certified Professional in Healthcare Quality (CPHQ), Lean/Six Sigma, or equivalent.u2022 Experience with OASIS or home health outcome measurement and coding.u2022 Prior supervisory or project leadership pensation & Work Conditionsu2022 Pay band (Columbus market guidance): $28.00--$40.00 per hour depending on experience and certifications.u2022 Typical schedule: Monday--Friday with occasional on-call or survey support; hybrid work model possible.u2022 Role requires travel to client homes, field offices, and survey sites.How to ApplyEmail resume and cover letter to or call 614-585-8008 for more information.
