Position Profile Hospice of the Valley's Arizona Palliative Home Care (AZPHC) team provides support and oversight to seriously ill patients struggling with daily living and disease management. Palliative care focuses on the patient's physical, psychological and spiritual needs. The goal is to achieve the best quality of life available by relieving suffering and controlling pain and symptoms. The AZPHC social worker provides psychosocial support to patients and families by providing skilled social work intervention to include, but not limited to providing resources, referrals, education, supportive counseling, discharge planning case coordination and education. Responsibilities u00a7 Develops and maintains therapeutic relationships. u00a7 Provides comprehensive psychosocial assessments. u00a7 Develops and maintains comprehensive plan of care. u00a7 Provides supportive counseling. u00a7 Facilitates smooth transition from care environments. u00a7 Mobilizes community resources. u00a7 Provides effective patient/family/caregiver teaching. u00a7 Supports the death/dying process. u00a7 Collaborates in providing patient care. u00a7 Creates timely and accurate documentation. u00a7 Assures patient safety. u00a7 Utilizes resources effectively and efficiently. u00a7 Adheres to HOV standards and facilitates continuously improved processes/services. u00a7 Maintains and enhances professional skills. u00a7 Adheres to high standards of personal and professional conduct. Minimum Qualifications u00a7 Bachelor's degree in social work from an accredited school and a minimum of 3-5 years palliative care or hospice experience. u00a7 Must have demonstrated ability to assess emotional needs, as well as social needs of patients and their families. u00a7 Requires the ability to administer psychological/emotional assessment instrument and define treatment planning from results. u00a7 Excellent communication and interpersonal skills. u00a7 Ability to effectively collaborate with interdisciplinary team in providing patient care. u00a7 Solution driven, creative and resourceful problem solving skills. u00a7 Ability to function independently and effectively anticipate needs and problems. u00a7 Skilled in organizing and prioritizing work. u00a7 Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through. u00a7 Must possess professional image, positive attitude, enthusiasm and self-motivation. u00a7 Comfortable in maintaining professional boundaries. u00a7 Proficient computer skills. u00a7 Broad understanding of community resources including those related to financial benefits. u00a7 Awareness and sensitivity to cultural diversity. u00a7 Knowledge of self-care, personal boundaries; abilities, limits and inner resources. u00a7 Current Arizona Driver's license and automobile insurance. Preferred Qualifications u00a7 Master's degree in Social Work from an accredited school and a minimum of one year social work experience in a health care setting (hospital, home health, hospice, behavioral health.) u00a7 Previous hospice experience. u00a7 LCSW or LMSW licensure. u00a7 Bilingual abilities a plus.
