Home Health Aide (HHA)

Responsibilities: Providespersonal care and rehabilitative services andassistsin activities of daily living as directed by the Director of Clinical Services or the supervising Registered Nurse. Participates in the development and implementation of a realistic goal-oriented plan of care. Provides nutritional support by preparing grocery lists and meals,assistingwith feeding and fluid intake, keeping record of nutritional intake. Assistsclients in administering medication. Assistswith personal care functions includinggrooming, bathing, toileting, dressing wounds, taking simple vital signmeasurementsand recording. Helping clientsexercise, move around the house and transfer them whenrequired. Accompanies clients to medical appointments. Assistsin socialization and recreation activities. Helps with personal housekeeping duties
