Job Description: Compensation : $ 130000 Job ID : 39655 Job Function: Provide comprehensive primary care services to adult patients, with a focus on a high Medicare population (ages 65+). Conduct routine physicals, manage chronic diseases, and address acute medical concerns. Collaborate with the transitioning physician during the handover period to ensure continuity of care. Maintain accurate and timely documentation in the EMR system. Coordinate care with specialists in gynecology, behavioral health, GI, and cardiology within the multispecialty group as needed. Deliver patient education on disease prevention, wellness, and treatment plans. Ensure compliance with hospital and regulatory standards of care. Provide high-quality, patient-centered care in both independent and collaborative settings. Job Requirements: Job Function: Active New York State license required. Training as an Adult or Family Nurse Practitioner. Board Eligible or Board Certified. 3 years of clinical experience.
