Ensure patient questions are answered, schedule appointments, process referrals and forms, generate documents, make copies of needed documents and provide excellent customer service so the patient leaves with a positive impression of VCC.Closely track patients that do not show up for appointments by calling, texting and sending lettersSchedule follow-up appointments with appropriate Clinicians during specific timeframesWork in partnership with the PSRs to check-in patients, verify insurance and collect formsAct as liaison at TCMC for delivering moms collecting pregnancy outcomes, birth histories and scheduling postpartum and newborn appointmentsProcess disability and FMLA formsProcess NT ultrasound referrals to TCMCRequest lab, imaging and specialist reports and scan into chartContact patients to schedule appointments (i.e. HEDIS lists, special reports and projects)Gather and send medical records to/from TCMC, Perinatologist and others as neededAssist in clinical data outcome reporting and participate in quality improvement projectsMinimumHigh school graduate or equivalentOne year related experienceBilingual English/SpanishValid California driver’s license, current vehicle insurance and reliable transportationRequired Skills/Knowledge/AbilitiesKnowledge of community health centers, and public assistance programs, state and federal regulations and managed care systemsExperience/familiarity with computers, Microsoft Office products (Word/Excel), business e-mail communication systems and internet search capabilitiesPay Range$21.00 - $22.50
