Job Description

Provides clinically appropriate biopsychosocial - spiritual care serving as patient population expert for members of interdisciplinary team. Conducts psychosocial assessment and diagnosis, makes treatment recommendations and engages patients and families to adherence by reducing or eliminating barriers. Assists with development , coordination , and oversight of clinical issues for Social Work staff.Responsibilities:Intervenes with patients and families regarding financial, social, emotional, educational, and environmental resources to mobilize resources to meet identified needs and develop an individualized plan of care.Uses social work/counseling training in growth and human development, family dynamics, cultural and religious diversity to provide interventions and manage risk by identifying potential adverse outcomes to work toward reducing and eliminating risks, then participates in discharge planning for complex patients.Assists the Medical Director and Administrator with engagement activities and participates in recruitment, interviewing and hiring process.Other duties as assigned.Education:Master's Degree in Social Work or Clinical Counseling from an accredited/approved school.Certifications:Current State Licensed Independent Social Worker Supervisor (LISW-S) or Licensed Professional Clinical Counselor required, which includes two years post graduate employment with clinical supervision.Advanced Clinical Certification in HCH - Mind/Body Trauma Resolution petencies:Requires critical thinking skills, decisive judgment and the ability to work unsupervised or with minimal supervision.Must be able to work in a stressful environment and take appropriate action.Work Experience:Five years Licensed Social Worker (LSW) or Clinical Counseling experience in healthcare and eligible to be Licensed Independently.Experience in utilization management, managed care and payor issues.Experience in psychosocial and therapeutic counseling.Experience in pre-acute and post-acute care and knowledge of community resources.Three years of practice with HCH - Mind/Body Trauma Resolution techniques.One year experience leading/managing a team.Physical Requirements:Requires direct patient contact.Requires extensive walking, telephone use, computer use, bending, reaching and lifting items weighing 20 pounds or less.Must be able to work well under pressure and maintain professional demeanor under adverse conditions.Extensive reading, writing, use of pagers, Fax machine, computers and copiers.Talking and hearing are essential in interviewing patients and families and in contacting outside agencies and in communication with hospitals personnel.Good visual acuity through normal or corrected vision.Personal Protective Equipment:Follows Standard Precautions when appropriate.Pay RangeMinimum Annual Salary: $69,600.00Maximum Annual Salary: $106,132.50The pay range displayed on this job posting reflects the anticipated range for new hires. A successful candidate’s actual compensation will be determined after taking factors into consideration such as the candidate’s work history, experience, skill set and education. The pay range displayed does not include any applicable pay practices (e.g., shift differentials, overtime, etc.). The pay range does not include the value of Cleveland Clinic’s benefits package (e.g., healthcare, dental and vision benefits, retirement savings account contributions, etc.).
