We are looking for an individual seeking a career opportunity with one of the largest employers within the Tampa Bay area. + Facility: BayCare Home Care + Location: Bradenton, FL + Status: Per Visit + Shift Hours: 8:00 AM - 5:00 PM + Weekend Work: Occasional + On Call: Occasional + Home visits in assigned service area: Manatee County, FL The Home Health Aide Per Visit Responsibilities include: + Working under the guidance and supervision of the staff nurse in providing personal care to the patients. + Following the patients' plan of care for completion of tasks assigned to a home health aide by the registered nurse or other appropriate skilled professional. + Maintaining an open communication process with the patient, representative (if any), caregivers, and family. + Demonstrating competency with assigned tasks. + Complies with infection prevention and control policies and procedures. + Reports changes in the patient condition; and honors patient rights. Required Certifications and Licensures: + CNA (Certified Nursing Assistant) or HHA certification from accredited 75-hour program + Driver's License State of Florida (Florida driver's license is required within 30 days of hire) + BLS (Basic Life Support) Education: + Preferred: High School Diploma or equivalent Experience: + Preferred 1 year Home Care Why join BayCare HomeCare?
