The Medical College of Wisconsin's Department ofMedicine - Division of General Internal Medicine is seeking a NursePractitioner or Physician Assistant to provide a broad range ofdiagnostic and therapeutic services for the patients of the Primary CareOutpatient Services located in Milwaukee, Wisconsin at FroedtertHospital Specialty Clinics. Our providers help patients understanddisease prevention and the treatment of conditions includingcardiovascular disease, endocrine disorders, cancer, and musculoskeletaldisease. We also support patients with chronic diseases by partneringwith other specialists to coordinate care. By working with our nursingstaff, social workers, and community health workers we can delivercomprehensive care at an elevated level. The Advanced Practice Provider(APP) works as a member of a multidisciplinary health care team who willcontribute to excellence in program administration, patient care,patient safety, research, quality improvement, and teaching. The IdealCandidate will be an experienced Primary Care APP who is looking to be apart of a fast-paced clinic and who wants to be part of a thriving andvery collaborative academic practice at the Medical College of Wisconsin(MCW). Areas of responsibility will include, but not be limited toproviding direct patient care to an adult population in an outpatientsetting. In this role you will: Collaborates with interdisciplinaryhealth care teams in diagnosis and treatment of conditions requiringgeneral medicine intervention in an outpatient environment as a memberof the Internal Medicine Clinic Health Care Team. The team isresponsible for providing management of primary care health andwellness, urgent care assessments and the treatment of co-morbidconditions such as but not limited to: diabetes, hypertension, heartfailure, asthma, and anemia in an adult population. Writes accurate andtimely progress notes, procedure notes, and/or admission and transferorders.
