JOB REQUIREMENTS: Position Description: The Medical College ofWisconsin's Department of Medicine, Division of Hematology and Oncologyis seeking a Nurse Practitioner or Physician Assistant to provideprimary or specialty medical care within defined scope of practice underthe supervision of a licensed physician and in accordance withMCW/FMLH/CHW policies and procedures. This position is located atFroedtert/MCW Benign Hematology clinic at FMLH with clinic coverage atother Froedtert locations as needed. The Advanced Practice Provider(APP) works as part of a multidisciplinary health care team, providing abroad range of diagnostic and therapeutic services for the patients ofthe outpatient services as a member of the Department ofMedicine/Division of Hematology and Oncology. The APP will have a strongfoundation in evidence based practice and ensure an environment of safequality care. In this role you will be responsible for the following:Collaborates with interdisciplinary health care teams in diagnosis andtreatment of conditions requiring Hematology/Oncology intervention inoutpatient environment as a member of the Benign Hematology outpatientservice. Writes accurate and timely progress notes, procedure notes,and/or admission and transfer orders. Order, perform, and interpretscreening and diagnostic procedures and tests, and appropriatelycommunicates and documents information. Privileged to perform proceduresbeyond core as identified by the Hematology/Oncology services.Facilitates and communicates with patient, family and staff to promotecontinuity of care across the continuum of care. Attend requiredtraining and meets key regulatory requirements, including those of theprivileging institution, CMS, and the Joint Commission. Evaluatesquality outcomes for the patients receiving care of this team throughparticipation in the development of quality indicators and outcomesdashboard. Acquires information on the most current development andtrends in the Physician Assistant field and/or Nurse Practitioner field,through educational programs, in-services, professional associates, andcurrent literature. Participates in ongoing clinical research projectsrelated to the field of hematology/oncology. Core Advanced PracticeResponsibilities: Performs comprehensive histories and physical exams,identifies normal and abnormal findings, including physical, cognitiveand psychosocial status. Orders and interprets laboratory and diagnostictests. Develops pharmaceutical and non-pharmaceutical treatment plans.Monitors effectiveness of plan of care.
