JOB REQUIREMENTS: JOB PURPOSE AND REPORTING STRUCTURE: Under thedirection of the Rapid Rehousing Program Manager, the Child Advocateprovides support and advocacy for previously homeless youth enrolled inthe Rapid Rehousing Program. The position ensures that youth receive thenecessary services and resources to achieve stability and independence.ESSENTIAL DUTIES AND RESPONSIBILITIES Include the following. Otherduties may be assigned. Ensure the social, physical, psychological, andemotional well-being of youth participating in the Rapid RehousingProgram. Prepare and maintain case plans on a quarterly basis. Conductinterviews with parents/guardians and youth to complete assessments anddevelop individualized treatment and service plans, including caseplanning, referrals, ongoing monitoring, and service coordination.Manage a caseload to provide appropriate resources and assist youth innavigating community services. Support youth in accessing medical andmental health services, medication management, activities of dailyliving (ADL) assistance, and AODA (alcohol and other drug abuse)services and supports. Communicate and report youth concerns toappropriate parties in a timely and professional manner. Collaboratewith clients on discharge planning beginning at the time of programadmission. Facilitate client transfers when services are no longerappropriate or when additional services are required; identify childrenwho may qualify for Wraparound Services. Coordinate transportation foryouth to and from appointments, including medical, mental health, andhousing-related visits. Attend and actively participate in departmentalmeetings and collaborative meetings with partner service agencies.Document all case management activities in Clarity, the HomelessManagement Information System (HMIS). Serve as the primary contact forlandlords, maintaining effective relationships and facilitating unitturnover and client placement as needed. Coordinate and oversee clientmove-outs, including meeting with clients to discuss move-out plans andexpectations. Assist in organizing social, recreational, and educationalactivities for program participants. Complete special projects asassigned. Maintain professional and technical competency by stayinginformed on industry trends and best practices. Complete and maintainall required documentation, records, and reports accurately and ply with all safety policies and work rules, maintaining a clean andorderly work environment.
