PositionDescription: The Advanced Practice Provider (APP) works as a member of amultidisciplinary health care team who will contribute to excellence inprogram administration, patient care, patient safety, research, qualityimprovement, and teaching. The Department of Medicine - Division ofGastroenterology is seeking a Nurse Practitioner or Physician Assistantto support a multidisciplinary health care team, providing a broad rangeof diagnostic and therapeutic services for the patients of theGI/Inflammatory Bowel Disease (IBD) Outpatient services team located inMilwaukee at Froedtert Hospital. The APP will be expected to work in ashared practice model with one or more faculty members, as well asindependently. The APP will be responsible for new and follow-up visits,in-basket management, TPN management and evaluating other acute patientissues. Learn more about: MCW GI & Hepatology Froedtert Hospital MCWAPPs Primary Functions/Responsibilities Collaborate withinterdisciplinary health care teams in diagnosis and treatment ofconditions requiring medical intervention in outpatient environment as amember of the gastroenterology team. Write accurate and timely progressnotes, procedure notes, and/or admission and transfer orders. Order,perform, and interpret screening and diagnostic procedures and tests,and appropriately communicate and document information. Privileged toperform procedures beyond core as identified by the gastroenterology andhepatology services. Facilitate and communicate with patient, family andstaff to promote continuity of care across the continuum of care. Attendrequired training and meets key regulatory requirements, including thoseof the privileging institution, CMS, and the Join Commission. Evaluatequality outcomes for the patients receiving care of this team throughparticipation in the development of quality indicators and outcomesdashboard.
