Personal Care Assistant

Employment Type: Full time Shift: Description: Position Purpose: The personal care assistant/worker is responsible for providing various care services necessary for attending to the personal needs, support, and health status of participants. Position Details: This position will primarily consist of home care assignments. However, occasional facility work will also be required. Position will require working one weekend a month and rotating holiday coverage. What you will do: u00b7 Engage in ongoing participant assessment and monitoring and report on services provided and the self-care abilities of participant. u00b7 Provide personal care assistance with activities of daily living, including toileting, showers, eating, and grooming. u00b7 Assist in the maintenance of physical activity, exercise, and walking. u00b7 Identification of safety hazards and emergency situations and initiating corrective action as able. u00b7 Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping. u00b7 Assist in identifying signs and symptoms indicating physical, emotional, or behavioral change in participants and reporting these signs to the center staff and/or supervisor u00b7 Escort participants to appointments as necessary. u00b7 Assist transportation staff with loading and unloading of participants from vehicles. Assist participants being transported in PO vehicles as needed. Minimum Qualifications: u00b7 High School Diploma or equivalent required. u00b7 Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring. u00b7 Experience in a long-term care facility or with a community-based geriatric program preferred. u00b7 Must be action-oriented, manage conflict well, be customer focused, have high decision quality and flexibility to adapt to ongoing change. u00b7 Ability to work with minimal supervision and exercise independent judgment. u00b7 Superior written and verbal communication and presentation skills appropriate for audience comprehension. u00b7 Valid drivers license and use of personal vehicle for home care visits u00b7 Excellent organizational skills and the ability to perform multiple duties and functions related to daily operations while maintaining excellent customer service. u00b7 Position may require 10% travel within the Centers geographic region and use of personal vehicle.
