Are you living your life's work? Summary: + Penn Presbyterian Medical Center is seeking a full-time Clinical Pharmacy Specialist, Opioid Stewardship. This position is responsible for provision of comprehensive medication management to patient populations across the institution with focus on pain management, palliative care, and substance use disorder. This pharmacist serves as a resource to Penn Presbyterian Medical Center on best practices, evidence-based guidelines, emerging therapies, transitions of care, pharmacologic pain management methods, optimal prescribing practices, effective medication reconciliation, and other pharmaceutical services.. This pharmacist collaborates on hospital and health system committees, workgroups, and initiatives and the position reports to the Director, Professional Practice. The successful candidate will have demonstrated contributions to pharmacy practice, leadership skills, and a personal commitment to advancing the profession.Responsibilities:u2022 Participate in patient care collaborative interactions with interdisciplinary colleagues, clinical pharmacists, residents, and students. Monitors medication therapy for appropriate selection, use, dosing, dosage form, route, therapeutic duplication and drug/dietary/disease state interactions and documents pharmacist interventions. Assists in navigation of drug shortages to limit impact on patient care.u2022 Ensure continuity of care through transition in care activities for patients including education and access to medication in collaboration with the ambulatory pharmacy.u2022 Provide educational in-services, newsletters, online material, and other media for health professional updates on pharmacotherapy topics. Develop competency assessments in pharmacotherapy management practices through experiential and didactic training for pharmacists.u2022 Serve in a preceptor/mentor role for pharmacy residents, pharmacy clerkship students, and pharmacists.u2022 Participate as an appointed member of the PPPMC Pharmacy and Therapeutics Committee and other hospital and health system committee memberships to support new clinical initiatives. Contribute to assessment and selection of medications for hospital formulary.u2022 Collaborate with multidisciplinary teams to ensure appropriate therapeutic uses of medication. Help maintain formulary adherence by prescribers.u2022 Develop and implement policies and procedures that support evidence-based treatment guidelines and clinical care plans for pharmacotherapy management. Ensure that existing pharmacy policies, guidelines, and educational papers pertaining to pharmacotherapy are current with the latest clinical advances and standards of practice.u2022 Develop clinical decision support tools relative to pharmacotherapy management. Provides operational support and recommendations for safe and effective use of information technology for pharmacotherapy management.u2022 Implement and maintain clinical pharmacy programs that support meeting Medicare Core Measures. Effectively utilize information technology to enhance pharmacotherapy management through surveillance, utilization, and outcome reporting. Contribute to organizational performance improvement by recommending changes for improving services to the pharmacy leadership team.u2022 Develop and conduct target drug programs and Drug Use Evaluations (DUEs) for selected pharmacotherapeutic agents. Report findings to Pharmacy and Therapeutics Committee.Credentials, Education, and Experience:u2022 Current Registered Pharmacist license in the Commonwealth of Pennsylvania (required)u2022 Board certification in Pain Management Pharmacy OR eligible for board certification within 1-year of hire (preferred)u2022 Doctor of Pharmacy Degree from an accredited College of Pharmacy and completion of a Pharmacy Practice Residency (PGY1) or equivalent experience (required)u2022 Successful completion of a PGY-2 Specialty Residency in Pain Management and Palliative Care Pharmacy OR two years of experience as a licensed pharmacist in a hospital with >50% of time spent with providing pharmaceutical care for pain management and palliative care patient populations and disease states (preferred)u2022 Experience precepting students and residents (preferred) We believe that the best care for our patients starts with the best care for our employees.
