Responsibilities + Assures that all consumers receive clinically sound triage/referral and ongoing consumer services for behavioral health needs. + Collects information necessary to develop an accurate assessment of each consumer + Participates in the development of the consumer treatment plan. + Provides mentoring, individual and group support, and teaches self-advocacy and motivation through feedback. + Provides community outreach and sustained efforts to engage individual clients. + Provides linkage to mental health services and programs including but not limited to psychiatric services, medication management, and representative payee. + Evaluate the consumeru2019s treatment plan through follow-up assessment and recommends adjustments based on clinical findings. Qualifications Education: + Requires GED or High School Diploma + Completion of, or willingness to complete within 6 months (after date of hire) approved Peer Support Training Model + Must have lived experience with a mental health, or behavioral health condition and in recovery + Be willing to apporiately share elements of their experience with others in a professional and supportive manner License(s)/Certification(s): + Crisis Prevention Institute (CPI) training within 6 months of hire. + Valid driveru2019s license when driving any vehicle for work-related reasons. Experience: + 1 year of experience in providing mental health or human services Specific offers are determined by various factors , such as experience, skills, internal equity, and other business needs.
