This position involves assisting clients in achieving their personal goals and fostering independence. The Residential Facilitator will work under the supervision of the Clinical Director to ensure the well-being and growth of individuals served. The expectation is that this position will be in-person, with the possibility of occasional remote work as necessary.u00a0Clinical Responsibilities:u00a0The Residential Facilitator will provide care coordination, resource management, and direct support to individuals in a residential setting. This includes assisting with various activities of daily living, medication monitoring, socialization, transportation, and advocacy. The Residential Facilitator will work as part of a collaborative service team to develop and implement individual service plans based on the assessed needs of the clients.u00a0The residential facilitators are required to assist with cleaning and light upkeep of the program.Job Duties:u00a0u00b7u00a0u00a0u00a0u00a0u00a0u00a0u00a0u00a0 Direct Support: Provide direct support and assistance to individuals in the areas of personal care, housekeeping, life skills, recreation and more.
