Job Summary:u00a0The licensed practical nurse may perform selected acts in accordance with The Nurse Practice Act, including the administration of treatments and medications in the care of the ill, injured, or infirm, the maintenance of health, and prevention of illness, under the direction of a registered nurse. Provides service in accordance with the Agency philosophy, standards and policies.Essential Functions:- Provides services in accordance with agency policies- Assists the RN to perform specialized procedures and carry out the Plan of Care- Preparing clinical and progress notes within the specified time frame of the Agency (Documentation created and completed the day of visit)- Assists the RN in performing specialized procedures and duties including; preparing equipment and materials for treatments, observing aseptic technique, assisting the patient and family member in learning the appropriate self-care techniques- Demonstrates knowledge of medications and their correct administration based on age of the patient and patient's clinical position- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors- Functions as a team member demonstrating collaboration with, and responsibility to patient, physician, interdisciplinary team- Performs patient care responsibilities considering needs to the standard of care for patient's age- Promotes patient/family/caregiver education using various verbal and written communication techniques that take into account the patient's/family's cultural, ethnic and/or personal needs or preferences- Attends 80% of mandatory in-services and case conferences to discuss multidisciplinary team responsibilities and patient progress- Collaborates with the Case Manager on upcoming Supervisory Visits (every 30 days)Professional Requirements:- Adheres to dress code, appearance is neat and clean- Adheres to legal practice standards, including applicable federal, state and local regulations and standards- Completes annual education requirements- Maintains patient confidentiality at all times- Demonstrates effective time management and organizational skills; provides patient visits as scheduled, completes patient care within designated time- Attends annual review and in-services, as scheduled- Maintains currency of professional knowledge by participating in continuing education offerings- Assures continued competency by maintaining and updating professional performance, knowledge and skills- Actively participates in performance improvement and continuous quality improvement activities- Practices in a manner consistent with the professional and Agency code of ethics- Represents the organization in a positive and professional manner- Complies with all organizational policies regarding ethical business practice- Communicates the mission, ethics and goals of the organization- Demonstrates effective time management and organizational skills; provides patient visits as scheduled; completes patient care within the designated timeRegulatory Requirements:- Graduate of an accredited nursing program. Current state licensure as a LPN- One (1) year experience in a clinical care setting, home health preferred within the last 24 months- Six (6) months home care experience within the last 12 months of hire- Valid state driver's license and reliable automobile- Proof of current automobile insurance- Current CPR and Hepatitis consent/declination- Flexible, yet organized, with ability to exercise sound judgment
