Description Hourly Pay Rate: $23 Shift: Monday-Friday 8:30am-4:30pm The Service Coordinator will provide general case management and referral services and link ESSHI qualifying tenants within the project to supportive services or medical services provided by public agencies or private practitioners within the general community. The service coordinator will assess service needs, determine the eligibility for public services, and coordinate the delivery of service with the appropriate state and local agencies. ESSENTIAL FUNCTIONS/RESPONSIBLITIES:u2022 Provides linkage and referral to services for ESSHI qualifying tenants.u2022 Coordinate appropriate benefits, entitlements and services.u2022 Provides evaluation of health needs, psychological and social needs and link to needed services.u2022 Provide formal case management with assessment and development of an individualized service plan with periodic re-assessment, monitoring, discharge and arranging for services.u2022 Maintains an up to date knowledge of senior issues including state entitlement programs, changes/revisions regarding Case Management and other appropriate community programs.u2022 Participates in case conferences/service plan meetings at a minimum of 4 times a year and specific training programs/services with public and private agencies. u2022 Provides information and assistance to tenants and participates in tenant move outs as needed.u2022 Annual Training is required in the areas of aging process, elder care, disability services, federal/state entitlement programs, legal and liability issues, drug and alcohol use and abuse by the elderly and mental health issues. u2022 Maintains confidentiality and process, maintains and utilizes potentially sensitive information in accordance with agency and departmental policies.u2022 Provide representation for benefit eligibility interviews u2013 ie. Medicaid, Food Stamps, Medicare, Social Security etc.u2022 Responsible for oversight of students, volunteers, aides or other individuals as assignedu2022 Provide eviction prevention activities.u2022 Conduct bi-annual surveys to assess resident needs and interests.u2022 Provide Information on site 12 times a year from community agencies/members including but not limited to budgeting, legal, credit counseling, tax preparation, etc. u2022 Advocate on behalf of tenant and family.u2022 Educate tenants, families, and staff on available Community Resources. u2022 Acts as a liaison between community agencies, tenants and families u2022 Provides on-going monitoring of service delivery u2022 Completes all required documentation including all tenant related services provided.u2022 Provides crisis management in services as needed.u2022 Compile and maintains resource materials.u2022 Complies with all agency policies and procedures.u2022 Other duties as assigned. MINIMUM QUALIFICATIONS:u2022 Bacheloru2019s degree with a preference to a background in gerontology, psychology or counseling.u2022 Service Coordinator/Case Manager experience with senior population preferred.u2022 Working knowledge of aging process and awareness of community services and programs preferredu2022 Ability to travel to various locations located in but not limited to Erie, Niagara, Chautauqua and Monroe County.u2022 Ability to organize, plan and manage caseloadsu2022 Must have sound judgment, flexibility, dependability and good writing skillsu2022 Ability to handle multiple tasks simultaneously SUPERVISORY RESPONSIBLILITES: u2022 Responsible for oversight of students, volunteers, residential aide or other individuals as assigned.
