The Case Manager assists clients to attain his/her clinical treatment plan goals in outpatient and other levels of care and works to mitigate barriers to success. Responsibilities include: Provide service coordination and linkages to the community Provides outreach services to clients, delivering mobile services in a number of community or institutional settings including inpatient units, homes, day programs, etc. Assist clients in obtaining benefits, housing and health care as needed Provide crisis support and participate in the development, revision and utilization of crisis prevention and/or safety plans Encourages and facilitates utilization of clients’ natural support systems as well as recovery-oriented, peer support and self-help supports and services Will assist in data collection and tracking for grant evaluation Requirements • Bachelor’s degree from an accredited educational institution or at least three years of relevant work or lived experience that is acceptable by MBHP waiver • Ability to pass certification for CPR, First Aid • Valid driver’s license; reliable transportation for business use • Ability to counsel and support clients with psychiatric and/or substance use disorder diagnoses • Demonstrated knowledge of trauma-informed care • Lived experience relative to psychiatric and/or substance use disorder preferred *Recovery Coaches welcome! * bilingual encouraged to apply: Spanish/English Qualifications Skills Preferred Bilingual Expert Behaviors : Motivations : Education Required Bachelors or better in Behavioral Sciences or related field. Preferred High School or better.
