Social Worker

Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Responsibilities Total Rewards of a Allied Health Professional Duties included but not limited to: Psychosocial Assessment: The social worker will complete a psychosocial assessment of the Veteran, family, economic, community, and psychosocial support systems according to program SOP. Case Management: The social worker will function as a case manager for Veterans in the MHICM program performing duties to facilitate independent community living by coordinating mental health and community care. MHICM Consults: The social worker will respond to consults as determined by the program coordinator by utilizing a screening tool and other measurement devices while assessing the Veteran s emotional, functional, and psychological condition. Treatment Planning/Goal Setting: The social worker is responsible for contributing to the treatment plan and setting achievable treatment goals with the Veteran/family in collaboration with the MHICM interdisciplinary treatment team members. Referral to service providers: Throughout the course of treatment, the social worker is the subject matter expert on VA and/or community resources. Advocacy: The social worker will act as an advocate when it serves the best interest of the Veteran/family. Crisis Intervention: The social worker is experienced in making rapid assessments and developing crisis management or suicide prevention safety plans. Education/Health promotion and prevention: The social worker will provide education related to VA and community resources, entitlements, Advance Directives/Living Will and will refer Veterans/families to the appropriate interdisciplinary team member for identified health education needs. Use of Government Vehicle to Transport Veterans: This position requires the ability to accompany and transport Veterans to appointments and other community resources as needed, whether medical, housing, or other treatment plan goal appointments through the use of a government vehicle. Work Schedule: Full-time, Monday through Friday; 8:00 a.m. to 4:30 p.m. Recruitment Incentive (Sign-on Bonus): Not Authorized. Permanent Change of Station (Relocation Assistance): Not Authorized.
