GENERAL SUMMARY: Works collaboratively with the Medical Director of the Henry Ford Health Center for Vision Rehabilitation to provide individuals with vision impairment opportunities for increased safety, independence and quality of life via world-class low vision care while achieving office productivity and performance goals. Directly reports to the Chair, Department of Ophthalmology and Eye Care Services. PRINCIPLE DUTIES AND RESPONSIBILITIES: u00b7 Comprehensive low vision case management: u00b7 Provide low vision consultations u00b7 Interdisciplinary collaboration u00a7 With HFH Center for Vision Rehabilitation occupational therapists u00a7 With other vision rehabilitation entities or professionals - including but not limited to agencies for the blind and visually impaired, orientation and mobility specialists, social workers, and certified driving rehabilitation specialists. u00b7 Specialty contact lens fitting if necessary to maximize visual function u00b7 Work with the Medical Director of Vision rehabilitation to maintain proper professional staffing and coverage within the clinic. u00b7 Professional growth u2013 Complete Michigan Optometric Low Vision Certification and American Academy of Optometry Fellowship u00b7 Compliance with continuing education requirements to maintain a license to practice optometry in the State of Michigan. u00b7 Support ancillary staff training programs u00b7 Support practice building programs. u00b7 Develop and support new relationships both inside and outside of HFH u00b7 Attend regularly scheduled office and staff meetings u00b7 Act as a role model for other staff u00b7 Embrace best practices to ensure that we are providing the highest possible level of professional care. + EDUCATION/EXPERIENCE REQUIRED: + Successful completion of the program from a school of Optometry. + Previous direct patient care experience. + Experience in low vision rehabilitation, preferred CERTIFICATIONS/LICENSURES REQUIRED: + Licensed as an Optometrist in the State of Michigan.
