Job Description: What Youu2019ll Do: The occupational therapist plans, organizes, collaborates and directs home care services. They are experienced in occupational therapy, with emphasis on community health education/experience. The professional therapist builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities in accordance with home care regulations and payer guidelines. More Specifically: u2022 Assumes responsibility to coordinate patient care for assigned temporary caseload including but not limited to: appropriate delegation to COTA staff and supervision provided by COTA staff, patient condition change visits, and completion of OASIS time points. Accepts accountability for patient satisfaction scores and quality of care outcome measure results. u2022 Completes and documents an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).Develops a patient specific plan of care, which establishes goals incorporating therapeutic, preventive, and rehabilitative actions. Includes the patient and the family in the planning process. u2022 Initiates the plan of care and makes necessary revisions as patient status and needs change. u2022 Initiates appropriate preventive and rehabilitative therapy procedures. Administers medications and treatments as prescribed by the physician. u2022 Counsels the patient and family in meeting therapy and related needs. u2022 Respects and supports patient and familyu2019s cultural beliefs and values, individual needs, health goals and treatment preferences. u2022 Provides health care instructions to the patient as appropriate per assessment and plan of care. u2022 Instructs, supervises and evaluates home health aide care provided every two (2) weeks as necessary. u2022 Helps manage episode of care or coordination of care with the primary disciplines in accordance with evolving occupational therapy needs and continued eligibility for services. u2022 Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient. u2022 Communicates with the physician regarding the patientu2019s needs and reports any changes in the patientu2019s condition; obtains/receives physicianu2019s orders as required. u2022 Attends regular case conferences and team meetings per Clinical Manager and as needed. u2022 Communicates with team COTA when delegating appropriate visits. u2022 Communicates with other team disciplines and community providers to coordinate the care plan (i.e. insurance case managers, high risk case managers, elder services, protective service, etc). What Youu2019ll Need: u2022 Graduate of an accredited program in Occupational Therapy. u2022 Licensed, certified Occupational Therapist with current licensure to practice therapy in the state of Massachusetts u2022 Active American Health Association BLS u2022 Minimum 2 yearu2019s experience preferred u2022 Oasis experience preferred What Youu2019ll Get: u2022 A highly competitive salary & benefits package, including generous PTO, 403(b), and tuition reimbursement u2022 A reasonable geographic territory with strong clinical support resources u2022 A highly inclusive, diverse team that values the input of all staff to provide excellent patient care Pay Range: $64,064.00 USD - $86,221.60 USD The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
