Demonstrated experience with each of the required areas of expertise in section B. Interpersonal skills to successfully interact with patients, families, hospital staff, community agencies. C. CTRS, ATR or BC, as specified by specific discipline professional certification guidelines. Registered Occupational therapists, Certified Occupational Therapy Assistants, and Registered Physical Therapists may also be considered with appropriate supervision and clinical competence. D. Bacheloru2019s Degree in Recreation Therapy, Music Therapy, Masteru2019s Degree in Art Therapy, or Associateu2019s degree in Physical or Occupation Therapy. E. CPI within 90 days of hire and then every calendar year F. American Red Cross or American Heart Association Basic Life Support Course (BLS or BCLS) and Certification Within Two Weeks of Hire Degree of Supervision Required: Involves general guidance and direction by Department Director. The employee will be expected to perform most job duties independently and in accordance with established departmental and hospital policies and procedures. Position Summary: Position Responsibilities Include but are Not Limited to: 1. Provide activities for patients admitted to the program which include following program group schedule and provide set holiday activities on provided dates. Provide support and or participation for after hour activities especially on weekends. 2. Participates as a member of the multidisciplinary treatment team. Attends set treatment team meetings and progress reviews as evidence by signature on treatment planning update forms. 3. Conducts individual activity assessments on each patient within given timeframes. 4. Makes a written summary of initial assessment, evaluation, treatment and progress as evidenced by initial assessment forms completed in the medical record and documentation in the progress notes. 5. Document patient progress with therapeutic activities in patientu2019s medical record. 6. Request needed supplies for activity department. Request should be made in compliance with the hospitals purchasing department guidelines. 7. Keeps equipment and supplies safe for use and in working order. 8. Performs other duties and special projects as required and approved by Program Director. 9. Attends required training and continuing education as evidenced by attendance forms that will be added to personnel file. 10. Completes all competency requirements as evidenced by completion forms, these will be completed annually or per hospital requirements. 11. Conducts individual, group and family sessions for patients as indicated patient treatment plan. 12. Facilitate identified support groups to meet the needs of patients/families as indicated by medical record. 13. Participates as a member of the multidisciplinary treatment team. 14. Participates in the QA process as evidenced by audits and attendance records. 15. Participates in development of master treatment plans and updates. 16. Participates in discharge planning and aftercare planning. 17. Makes necessary contacts with community resources to ensure continuity of care. 18. Documents according to policies and procedures. 19. Gathers and collects pertinent clinical information. 20. Maintains accurate listing of all patients on the behavioral health unit. 21. Provides education and information about the inpatient unit to patients and families. 22. Cultivate positive relationships with and maintain an environment of collaboration and cooperation at all levels of the organization, including the Medical and clinical staff, referral sources and community partners. 23. Positively and professionally represents the unit with internal and external customers, other shareholders, and the public. 24. Meets position requirements and performs essential function. 25. Provides timely, accurate and complete reports as requested. 26. Demonstrates ability to set priorities. 27. Demonstrates good judgment in handling situations not covered by verbal or written instructions. 28. Follows established policy and procedures in performing job functions. 29. Interacts and communicates with others in a way that promotes a positive and cooperative work environment. 30. Channels concerns appropriately. 31. Provides input, suggestions and support appropriately. 32. Demonstrates a thorough knowledge of work practices. 33. Ensures patient rights and confidentiality. 34. Provides resources and support for patients before, during and after treatment. 35. Performs other duties as assigned by Program Director.
