Job Summary: u00b7 Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization. Responsibilities: u00b7 Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate. u00b7 Serves as a primary coordinator of all disciplines for well-coordinated patient care. u00b7 Notes and carries out physician and nursing orders. u00b7 Assesses and coordinates patient's discharge planning needs with members of the healthcare team. u00b7 Demonstrates the knowledge and skills necessary to assess patients and to identify physical, psychosocial, cultural, spiritual and age-related needs in order to provide individualized care to the patient. u00b7 Delegates aspects of care to other nursing personnel consistent with their level of education and expertise in order to meet patients' needs and to maximize staff performance. u00b7 Participates in daily interdisciplinary patient rounds. Demonstrates knowledge of patients' condition and is able to provide appropriate input to meet desired outcomes for the appropriate patient population. u00b7 Reflects ANA Scope and Standards of Nursing Practice (2015), ANA Code of Ethics (2015), and ANA Social Policy Statement (2015). u00b7 Develops, implements and revises individualized plans of care. u00b7 Participates in activities for professional growth and development outside of patient care. u00b7 Other Duties as assigned. Must have experience educating patients on lifestyle changes to improve cardiovascular health and prevent future heart problems. Experience using various exercise equipment, analyzing patient data, and collaborating with a multidisciplinary team of healthcare professionals. Qualifications u2022 Graduate of an accredited nursing school. u2022 Associate degree required; BSN preferred. u2022 Current Connecticut RN license. u2022 BLS certification by the American Heart Association. u2022 Excellent assessment and critical thinking skills. u2022 Must have experience providing care to patients recovering from heart conditions, such as heart attacks, heart surgery, or other cardiovascular events.
