Medical Director, Geriatric

Description Rochester Regional Health is seeking an experienced physician leader to serve as Medical Director for ElderONE, the region's Program of All-Inclusive Care for the Elderly (PACE). ElderONE provides comprehensive, coordinated care that helps frail older adults safely remain in their homes and communities.The Medical Director oversees clinical quality, regulatory compliance, and interdisciplinary team performance while also providing direct patient care. This leader partners closely with operational leadership to drive program growth, quality outcomes, and participant satisfaction.Key Responsibilities:Provide clinical leadership for all ElderONE/PACE sitesLead and mentor interdisciplinary care teamsEnsure compliance with CMS and NYSDOH PACE regulationsOversee care planning, quality initiatives, and clinical standardsContribute to strategic planning, program operations, and performance improvementQualifications:M.D. or D.O., board certified in Geriatrics, Internal Medicine, or Family Medicine (Geriatrics preferred)Eligible for NY medical licensureMinimum 5 years of relevant clinical experience; leadership experience strongly preferredStrong communication, collaboration, and team leadership skillsAbout ElderONE / PACEElderONE, sponsored by Rochester Regional Health, is the oldest PACE program in New York State. PACE provides comprehensive medical and social services to eligible older adults through an interdisciplinary team approach, enabling participants to remain safely at home rather than entering a nursing facility. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
