Description Rochester Regional Health is seeking an experienced physician leader to serve as Medical Director for ElderONE, the region's Program of All-Inclusive Care for the Elderly (PACE). ElderONE provides comprehensive, coordinated care that helps frail older adults safely remain in their homes and communities.The Medical Director oversees clinical quality, regulatory compliance, and interdisciplinary team performance while also providing direct patient care. This leader partners closely with operational leadership to drive program growth, quality outcomes, and participant satisfaction.Key Responsibilities:Provide clinical leadership for all ElderONE/PACE sitesLead and mentor interdisciplinary care teamsEnsure compliance with CMS and NYSDOH PACE regulationsOversee care planning, quality initiatives, and clinical standardsContribute to strategic planning, program operations, and performance improvementQualifications:M.D. or D.O., board certified in Geriatrics, Internal Medicine, or Family Medicine (Geriatrics preferred)Eligible for NY medical licensureMinimum 5 years of relevant clinical experience; leadership experience strongly preferredStrong communication, collaboration, and team leadership skillsAbout ElderONE / PACEElderONE, sponsored by Rochester Regional Health, is the oldest PACE program in New York State. PACE provides comprehensive medical and social services to eligible older adults through an interdisciplinary team approach, enabling participants to remain safely at home rather than entering a nursing facility. Services include primary and specialty care, rehabilitation, transportation, social engagement, and 24/7 clinical support.About Rochester Regional HealthRochester Regional Health is one of the largest integrated health systems in New York State, serving communities across Western New York and the Finger Lakes. The system includes five hospitals, over 300 practice locations, leading home-based and community care programs, and a shared commitment to clinical excellence, innovation, and compassionate service.PHYSICAL REQUIREMENTS: L - Light Work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly; requires occasional walking, standing or squatting.PAY RANGE: $250,000.00 - $300,000.00 The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
