Job Description

The Discharge Planning Assistant is an active member of the caretransitions team, working collaboratively with the Inpatient RN CaseManagers. The Discharge Planning Assistant is responsible for performingadministrative and operational functions needed in organizing the CareTransitions Department. Reporting to Care Transitions leadership, theDischarge Planning Assistant will provide direct administrative supportto the discharge planning process. This position will spend time onclinical units and directly interact with patients and their families,clinical and ancillary hospital staff, and other internal and externalcustomers. Job Description: Duties/Responsibilities: Supports the caretransitions discharge functions by entering referrals as directed.Supports the gathering of clinical information for payor authorizations.Provides required documentation to third-party vendors in order tosecure post-acute transition services Provides direct support to the RNCase Manager in the clinical units Verifies patient demographics,including address, telephone number, and insurance. Verifies thepresence of health care proxy. Distributes regulatory notices andcompletes all required documentation. Assists with securing patientresources as requested. Assists with post-acute referral process andmonitors status and follows up on screening determinations, asnecessary. Identifies in-network providers, for example, VNA, DMEvendors, rehabilitation facilities, etc. Collaborates with the RN CaseManager to facilitate timely discharge to the next level of care.Demonstrates thorough knowledge of various computer/information systemsto perform assigned duties, including but not limited to Careport, EMR,and Outlook Collects/inputs data or information in appropriate databasesas indicated by the Care Transitions leadership. Provides documentationto patient/family/caregiver under the direction of RN Case dependently identifies and communicates any problems or issues thataffect departmental goals and outcomes. Performs routine tasks such asopening mail, providing accurate and appropriate information to callers,and other clerical duties. Education: Required: High School Diploma orGED Preferred: Associates Degree or Bachelor?s Degree Experience:Required: Excellent computer skills, including proficiency with variouscomputer systems to performed perform assigned duties, including but notlimited to Meditech, Epic, Cerner, MS Outlook Strong communication,interpersonal, and customer service skills Excellent assessment andproblem-solving skills
