Madison College ClinicThis position is eligible for a $3000 INCENTIVE for joining GHC(Amount prorated based on FTE) The Care Team Specialist (CTS)participates as a member of a care team and supports high qualitypatient care including engaging patients in maintaining their physicaland mental well-being. The Care Team Specialist performs technical,clinical, and administrative tasks which contribute to the smoothworkflow and quality patient care experience in the clinic.Responsibilities include, but are not limited to pre-visit preparation,monitoring patient flow, rooming patients, obtaining patient historiesand vital signs, managing certain test results following protocols,supporting Epic in-basket workflow, returning phone calls, and assistingwith medical examinations and procedures. The Care Team Specialistassists and collaborates with the Registered Nurses and MedicalProviders in the management and coordination of patient care. The CTSstaff will participate in the review of team/provider in-baskets and MyChart messages for the assigned care team. Practitioner and RegisteredNurse staff may delegate assignments to the Care Team Specialist. CTSstaff at GHC-SCW will have successfully completed medical assistanttraining, licensed practical nurse training, Paramedic training, or EMTtraining from an accredited program.
