Job Description Job Summary: The Medication Reconciliation Technician plays a critical role in ensuring patient safety by accurately collecting, reviewing, and documenting medication histories. This position supports pharmacists and other healthcare providers in identifying and resolving discrepancies in medication records during transitions of care, such as admission, transfer, and discharge. ________________________________________ Key Responsibilities: u2003Obtain and document accurate medication histories from patients upon admission. u2003Going into patients rooms with doctors and nurses to obtain history (patient facing role) u2003Review and compare current medications with newly prescribed ones to identify discrepancies. u2003Collaborate with pharmacists, physicians, nurses, and outpatient pharmacies to clarify medication information. u2003Assist in updating allergy information and medication lists in electronic health records (EHR). u2003Educate patients and caregivers (families) on medication changes, usage, and potential side effects. u2003Ensure medication lists are accurate during transitions of care. u2003Update accurate medications in EMR (epic) u2003Calling pharmacies to cross check medications u2003Support inventory control and medication labeling as needed.
