Skilled Caregiver (RN/LPN/CNA/HHA) - Rural Des Moines

Job Description:As a Skilled Caregiver, you will provide professional home care services in accordance with individualized care plans. This role requires strong clinical knowledge, compassion, and adaptability, as you may work with clients who have complex needs.Key Responsibilities:- Follow individualized care plans as directed by supervising nurse or physician.- Assist clients with personal care, mobility, and activities of daily living.- Administer medications and monitor vital signs (RN/LPN scope).- Support wound care, oxygen use, and other specialized treatments as appropriate to license.- Encourage client independence while ensuring safety.- Provide accurate, timely documentation of all client visits.- Communicate effectively with families, supervisors, and healthcare team members.Qualifications:- Active Iowa RN, LPN, CNA, or HHA certification (depending on position).- Current CPR certification (employer provided if not current).- Recent TB test or ability to obtain before hire.- Reliable transportation for travel between client homes in rural Polk County/Des Moines.- Ability to lift up to 30 lbs. and assist with transfers.- Strong communication and documentation skills.Benefits and Perks:- Flexible scheduling (full-time, part-time, PRN available).- Competitive starting pay, based on role and experience.- Gas/mileage reimbursement for client travel.- One-on-one home care assignments (no rushed facility workload).- Direct impacthelp families in your own Iowa community.Note on Training:Employees are responsible for maintaining required state certifications and continuing education (RN/LPN/CNA/HHA).
