JOB REQUIREMENTS: Position Description: The Department of Medicine -Division of Hematology and Oncology is seeking a Nurse Practitioner orPhysician Assistant to support the inpatient services located inMilwaukee, Wisconsin at Froedtert Hospital, Center for Advanced Care,and other inpatient units. The Advanced Practice Provider (APP) works asa member of a multidisciplinary health care team who will contribute toexcellence in program administration, patient care, patient safety,research, quality improvement, and teaching. PrimaryFunction/Responsibility: Collaborates with interdisciplinary health careteams in diagnosis and treatment of conditions requiringHematology/Oncology/BMT intervention in inpatient environment as amember of the HemOnc/BMT inpatient service. Evaluates quality outcomesfor the patients receiving care of this team through participation inthe development of quality indicators and outcomes dashboard.Facilitates and communicates with patient, family, and staff to promotecontinuity of care across the continuum of care. Evaluates qualityoutcomes for the patients receiving care of this team throughparticipation in the development of quality indicators and outcomesdashboard. Participates in ongoing clinical research projects related tothe field of hematology/oncology/BMT Core Responsibilities: Performscomprehensive histories and physical exams, identifies normal andabnormal findings, including physical, cognitive, and psychosocialstatus. Order, perform, and interpret screening and diagnosticprocedures and tests, and appropriately communicates and documentsinformation Develops pharmaceutical and non-pharmaceutical treatmentplans. Monitors effectiveness of plan of care. Educates patients ondisease and treatment plans. Performs procedures as trained andappropriately credentialed. Documents within the Electronic MedicalRecord (EMR) and completes other necessary documents. Collaboratesand/or consults with physicians and other health care providers asappropriate. Educates, counsels, and assists patients and familiesregarding current health problems, health maintenance activities andpreventive behaviors.
