In this role you will be able to apply your strong analysis, interpretation and interpersonal skills to support patients through complex issues.ResponsibilitiesYou will administer assessment tools and gather data for assessment, and to assess for optional needed servicesYou will interview families to determine social work needs of the participantYou will contact community resources to coordinate servicesYou will provide initial and on-going assessment of participant's social, emotional and cognitive status and needs, involving participant and other staff members in the assessmentYou will ensure accurate documentation is maintained for all assessments, treatment plans, and evaluationsRequirementsMaster's degree in Social Work requiredState social work licensure preferredMinimum one (1) year experience at Master's Level working with an Interdisciplinary Team, preferably in a post-acute settingExperience with geriatrics and disabled adults preferredUtilization Review experience preferredOur ValuesAs a MultiCare employee, we'll rely on you to reflect our core values of Respect, Integrity, Stewardship, Excellence, Collaboration, Kindness and Joy.
