Join our multidisciplinary care team and help provide critical support services to patients and their families in a variety of healthcare settings u2014 including acute care, outpatient, and long-term care. As a Medical Social Worker , you will deliver high-quality psychosocial care, engage in crisis intervention, assist with care transitions, and be a key resource for patient education and advocacy. What Youu2019ll Do: + Assess and address the psychosocial needs of patients and families. + Coordinate discharge planning , referrals , and protective services as needed. + Provide crisis intervention , pre-admission screening , and long-term care support . + Collaborate as part of a multidisciplinary team to support patient-centered care. + May participate in program development , professional training, and student supervision.
