The Wellness Coordinator for La Central provides psychoeducation and support, referrals and linkages to community resources for clients, ensuring Responsibilities include Responsible for planning and coordinating special events and holidays activities. The Counselor will also develop and implement weekly recreational activities including field trips, outings, arts & crafts, games, music and other hobbies. Schedule: Monday - Friday 9am - 5pm Essential Duties: + Conduct needs assessments + Establishing a network of community resources to facilitate wellness activities + Recommend appropriate activities, lead and co-lead activity groups and encourage clientsu2019 participation in community events + Provide wellness-related counseling and monitor clientsu2019 progress with their goals + Work collaboratively in an inter-disciplinary team to ensure comprehensive, client-centered services are delivered and documented Qualifications + Bacheloru2019s Degree in Human Services, Education or a related area + Licensed Practical Nurse (LPN) NYS license + Two (2) yearsu2019 work experience in benefits and entitlements with extensive knowledge of community resources + Experience with HIV/AIDS afflicted individuals, chronic mental illness, substance abuse individuals and their families a plus + Bilingual skills in English/Spanish a plus + Knowledge and hands-on experience in providing direct services related to the treatment and recovery of persons with HIV/AIDS, substance abuse, serious mental illness, and homelessness + Excellent oral and written communication skills; proficiency with MS Office and Google product suites + Ability to perform and manage in a team approach; excellent interpersonal skills + Ability to handle complex challenges and balance multiple demands on time and attention Comunilife provides equal employment opportunities to all employees and applicants for employment and prohibits discrimination and harassment of any type without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.
